Petropavlovsk-Sakhalinsk (Russan: Петропавловск-Сахальский) is the largest settlement on Sakhal, located on the west coast of the largest island.

Petropavlovsk-Sakhalinsk is a major town and the largest settlement of Sakhal, and a home to several buildings that appear nowhere else on the map. Under default settings, players never spawn in the town, but have to travel there instead.
The town consists largely of apartment blocks, industrial areas and has a very distinct port/harbor theme. It is built around a 270-degree bay, which the city surrounds on three sides. For a coastal town it has a low count of boat spawns, with a military-medical-heavy south district being the main attraction for survivors. In addition to the usual facilities found in major towns, Petropavlovsk is the only town in the map with a school building, as well as the only one with an amusement park.

- The town is based on the real-life coastal town of Petropavlovsk-Kamtšatski in the far-east region of Russia.
- The name of the city translates literally to "The city of Peter and Paul in Sakhal"
- In front of the amusement park, there is a bear statue that clearly states "Here begins Kamchatka" (Russian: здесь начинается Камчатка), referring to the real-life Kamchatka Peninsula that served as inspiration for Sakhal.
